Overview: JOB SUMMARY The Registered Nurse is a trained professional nurse who administers skilled nursing care to patients on an intermittent basis in the home. May be required to perform OASIS and assess patients within regulatory requirements of OASIS and timelines. This is performed in accordance with the physicians orders and under the direction and supervision of the Clinical Supervisor. May be required to perform admission, routine,or continuous care visits and assess patients within regulatory requirements. The Registered Nurse is responsible for overall coordination of the care delivered to her/his patient caseload. This is accomplished by making regular visits to patients wherever they reside, communicating regularly with the Interdisciplinary Team, medical director, identifying patient, family, caregiver needs and providing for those needs in accordance with the attending physicians orders and theplan of care. This position requires the ability to build rapport and communicate effectively with patients, families, physicians, facilities. Assesses assigned case load of home healthcare patients to identify the physical, psychosocialand environmental needs of patients as evidencedby documentation, clinical records, case conferences,team report, call-in logs, and on-site evaluations.Meets all regulatory and accreditation requirementsrelated to following the Plan of Care. Makes the initial evaluation and re-evaluates the patientsnursing needs during each visit. Makes follow upassessments according to OASIS and PPS regulatoryguidelines. Communicates significant findings, problems, andchanges in condition or environment to the clinicalsupervisor, the physician and/or other personnel involvedwith patient care. Reports unsafe condition to the appropriate ClinicalSupervisor and physician in a timely manner. Implements the plan for patient safety, using the patient,family, and community resources Participates in the implementation of the plan of care of an assigned case load to ensure quality, continuity of care,and achievement of patient outcomes. LICENSURE: Current State License as a Registered Nurse. Must have valid drivers license. EXPERIENCE: 1 year experience as a nurse in a clinical care setting or home healthenvironment preferred. SKILLS: Nursing skills as defined by accepted nursing standards. TRANSPORTATION: Reliable transportation and valid automobile insurance coverage. HEALTH STATUS: Meets all applicable agency policies and procedures related to healthscreening and required testing.are setting or home health environment preferred. Physical Demands: Complete required work on a daily basis. Cope with work involving ill, disabled, and dying patients. Sit, stand, bend, lift, and move intermittently.
